Ethos + Co | Facility Request & Agreement

Ethos is honored in your interest to our building. The mission of this space is to foster relationships, do good, and facilitate healthy conversations about life. Ethos cannot guarantee the reservation of the building for you and your event. By providing more information about your event, we will be able to determine if and how we can best accommodate your facility request. With space scheduling for Ethos Church, we are scheduling the facility 3-months out at a time. Please give our team 7-10 business days to get back to you.

Please include your setup and tear down timeframes as well. You can follow this format.
The building is booked for events for 4 hours and then bill hourly for additional hours.
Access to the building for the renter and outside vendors is limited to the agreed upon rental timeframe.

Setup: 2-3pm
Event: 3-5pm
Tear Down: 5-6pm

FACILITY POLICY & AGREEMENT

Once the "Facility Request and Agreement" form is completed, a team member will be in contact with you regarding the availability to host your event at the facility and a contract with rental rates.

Please review Ethos + Co.'s policies and agreements for use of the facility. If the client agrees to all of the policies, please initial in the available box for each policy and provide a digital signature.

The rental agreement is for the first-floor multipurpose room and paved parking area, and only for the agreed upon date and timeframe. The client will NOT be permitted access and use of the second-floor, food pantry, café kitchen and service area, and parking in the grass. The conference room may be requested for use. If the conference room is added to the facility use, there may be an additional fee based on use and necessary cleaning.

A security deposit of $100 per event holds your event in the calendar. The security deposit is NOT counted towards your facility use fee, and will be used towards any damages or cleaning necessary following events. If the security deposit is NOT necessary, it will be refunded to the client or the client will have the opportunity to donate it to the Food Pantry.

The facility use agreement does NOT cover the use of café equipment nor refrigerator, freezer and ice machine.

The client will be permitted to utilize the tvs to airplay remotely from the client’s personal computer or device. The tv cables, adaptors, or devices are not allowed to be disconnected from the tvs.

Chairs and tables in the main room may be used and moved by the client. If tables and chairs are not returned and organized to their proper place, there may be an “organizing fee” of $75. Any broken or damaged tables and chairs will be the responsibility of the client to financially replace for the exact product from the original vendor. Damage will be assessed by the Ethos Church Inc. team following the event, and may include but are not limited to; permanent stains, burns, scratches, or chips.

The client is allowed to bring in catered food, order-in food, potluck, or schedule a food truck. If you'd like to book a food truck for your event, please get approval from the Ethos + Co. manager, and the client agrees to commit to the financial responsibility to clean any stains or damages caused by the food truck/vehicle.

Decorations are allowed in the facility, however, no wall-mounted (no tape and thumbnail) decorations are allowed in the facility. The client is fully responsible to repair any and all damages caused by decorations. Glitter decoration or supplies is prohibited and not allowed inside the facility or on the grounds.

The client will be held responsible for any and all damage or loss to the facility, grounds, equipment, or property caused by the negligence or intentional misconduct of any participant or guest of the client. The client will be billed for any cleaning required, repair, and/or replacement costs for damages caused by participants or guests of the client. Damage to the facility includes but is not limited to, unintended use of doors, door crash bars, and windows, food ground into the floor, beverage spills on the floor, broken or damaged Ethos decorations, broken glass, damaged doors or door frames, stained or damaged chair pads, holes in drywall, cracked/scratched flooring, etc. Payment will be due from the client for such costs after the event and will be covered with the $100 security deposit and potential charge to the credit card if damages exceed the $100 deposit.

There will be NO open flames inside the building outside of birthday candles for a cake.

There will be NO smoking inside the building at any time or on the grounds.

There will be NO alcohol in or served within the facility or on the grounds.

Clients are required to bag all trash and place it in the dumpster. Trash and dumpster services are NOT included in the facility use agreement and fee. If the quantity of trash requires Ethos + Co to contract an extra dumpster pickup, the client will be billed for the service out of the security deposit.

The client agrees to clean the facility and returned to the condition in which they found it. The client agrees to and is not limited to the following; cleaning restrooms floors/toilet/sink, vacuuming and mopping the floors, and returning furniture to its original location.

In the situation where the client’s event necessitates additional cleaning from Ethos + Co. staff or an outside vendor, the client commits to financially reimburse Ethos for the time and fees.

The client agrees to hold Ethos Church Inc. as well as Ethos + Co. and their employees harmless for, from, and against any and all claims, actions, damages, liabilities, and expenses for the loss of property, life, personal injury, or damages to property arising directly or indirectly from or out of the event.

All cancellations must be received in writing in the Cancellation Addendum. If the event is canceled within 2 months of the event, the security deposit will be considered a donation to the Food Pantry and there will be no other amount due. The event date may be postponed with written notice between 2-4 months before the originally requested date at no cost so long as the facility is available on the date.

The Ethos Church Inc. and Ethos + Co. is not financially responsible in the event of: a National Emergency, Severe Weather Warning, or Act of God that forces a last-minute cancellation of the event. If there is a Government mandated shutdown, a full refund will be provided. Ethos will make every effort to reschedule your event in a timely and convenient manner. Therefore, all linens and China must be rented from a third-party source. Ethos Church Inc. and Ethos + Co. does not allow decorations to be stapled, taped, nailed, or any other substance to be affixed to any of the walls, floors, or ceilings in the rooms. Water glasses and all other China is not included at Ethos + Co. No glitter, confetti, birdseed, synthetic petals, or rice is allowed. All vendor items and personal items must be removed at the conclusion of your event or will become the property of Ethos Church Inc. and Ethos + Co. and may be disposed of or used, at the discretion of Ethos.

Organizational Users. User promises and warrants that it carries liability insurance with a minimum liability occurrence limit of $1,000,000. The User will provide a certificate of insurance to the Owner at least seven days prior to the date upon which the User begins to use the above described premises. The certificate of insurance will indicate that User has made Owner an “additional insured” on User’s policy with respect to the use by User of the above described premises.

I have read the deposit and cancellation policy. I understand and agree that I am responsible for remitting deposit payments according to the policy and that if the deposits are not received by each due date, the event may be subject to cancellation. I have read and understand the terms of this agreement. I understand that Ethos Church Inc. and Ethos + Co. is not responsible for food items taken off premises to be consumed at a later time by either the guests or myself after the event. I agree with the terms and conditions of this contract. I concur with the deposit policies and will follow through with them as such. Please obtain confirmation of the event with Ethos Church Inc. and Ethos + Co. I understand that the final event details are due 14 days prior to the event and late payments are not guaranteed and will result in an administrative fee being assessed. Ethos Church Inc. and Ethos + Co. reserves the right to change their policies as determined necessary.
